Frequent trips to the ER and hospital. The Advanced Care Group is here to help. Our sole purpose is to provide an extra layer of support for you and your family as you walk through your illness and to ensure that you receive the care you want, in the location that you want it, and at the time you need it.
The Advanced Care Group’s services are targeted toward patients suffering from serious illnesses such as cancer, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), kidney failure, liver failure, advanced dementia, traumatic injury and ALS. Our services are most appropriate for patients with advanced stages of these diseases, ie Stage IV cancer and/or patients who have earlier stages of these diseases but also have frequent visits to the hospital.
We remain focused on ensuring that you receive the care you want, in the location that you want it, and at the time you need it. The Advanced Care Group assigns each of its patients a specialized team of clinicians that includes a physician, nurse practitioner and nurse and – depending upon your needs and preferences – a social worker and chaplain. This team will stay the same throughout your entire time with The Advanced Care Group so that you will always know who to call.
Our service begins with an initial assessment. In this initial meeting, we will try to better understand your illness, your goals, any symptoms that are giving you difficulty, and the level of support you have from your family, friends and other caregivers. Based on this information, we will work with you to develop a care plan that is focused on your specific needs and goals.
Our clinical team will then walk with you through the course of your illness, helping ensure that your needs are met and your goals are achieved. We’ll be available to come to you when you need us. We will always be checking in with you to make sure you’re doing okay, and we’ll be there in the middle of the night if you need to call us in the event of a crisis. Although each care plan is different, most of our care plans include:

▪ Coordination with other medical professionals
▪ Communication with family members and other caregivers
▪ Coordination of in-home care and support services
▪ 24/7 support in the event of a crisis
