Advanced Nurse Practitioner

POSITION PURPOSE:
In collaboration with a physician, the Nurse Practitioner provides for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions for our transitional care/palliative care organization.

Transitional care is a combination of disease modification, primary care and management of distressful symptoms in individuals with severe illness—recently discharged from either an acute care facility (hospital), post-acute care facility (SNIF) or long-term care facility.

POSITION RESPONSIBILITIES:
•Maintains communication with primary care provider
•Provides continuity across time and space of multiple care levels
•Coordinates and maintains continuity with secondary services (home health, rehabilitation facilities)
•Refers to secondary providers for difficult and/or complex management issues (psychiatric illness)

POSITION QUALIFICATIONS & LICENSURE
Required Licenses/Certifications/Registration
•BLS Provider credentialed from the American Heart Association (AHA) obtained prior to hire date or job transfer date
•Nurse Practitioner specializing in Advanced Practice obtained prior to hire date or job transfer date
One or more of the following:
• Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "compact state" obtained prior to hire date or job transfer date.

POSITION EDUCATION:
Master's Level Degree or MSN from an accredited program approved by the State Board of Nursing program.

POSITION EXPERIENCE:
Minimum of three-five years of clinical nursing experience. One-two years of Nurse Practitioner experience preferred.
CCU or ICU experience highly recommended.
